Custom domain followers vs wordpress followers
-
Hey there :) I just changed my wordpress blog to a self hosted on at http://www.acleverheart.com, I’ve lost all my wordpress followers from the old blog and I’m not sure if the tags in my articles will show on WordPress since I’m using a self hosted site.
The blog I need help with is: (visible only to logged in users)

You can move your followers to the self-hosted WordPress. Jetpack integration would be required for it to work.
Follow the instructions on the following link:

Self hosted sites are not a part of the WordPress.COM community. They are free standing “islands” so to speak and the posts in them do NOT automatically post to the WordPress.COM topics pages.
To migrate subscribers install the JetPack plugin http://jetpack.me and follow these instructions please https://en.support.wordpress.com/moving-a-blog/moving-your-subscribers/#wordpress-com-to-self-hosted-site
I’ll also tag this thread so that Staff will move your stats for you.

Thank you! Does that jet pack plugin allow me to post to the wordpress.com topics pages? If not, would I need to maintain a hosted site and a self hosted site in order to get the traffic and community support from WordPress?

No. I repeat that no self hosted WordPress.ORG installs are part of the WordPress.COM hosted blog community. There is no way to have a foot in both camps, so to speak because if you duplicate content across domains both of your sites will suffer. Jetpack would allow your WordPress.com followers to get your posts in their Reader.
However, other users of WordPress.com would not be able to see your posts in ther Reader or its tags/topics.

You absolutely were. Just wanted to add that existing followers would still get the posts in their Reader.
