Custom FIeld page (with file upload)

  Hi.. I’m looking for a plugin/widget that will allow me to create a custom page with a 4-5 fields like name, picture (so there should be file upload). When a user fills and sends this information.. it should go to another page that simply lists is. I.e. like a guest book but with an image upload.

  Any such plugins ?
