Custom Form Fields

    Hi guys,
    I’m looking for a plugin that allows admin to choose custom form fields when creating new posts.

    Conditions:
    mandatory and non-mandatory custom form fields when starting new post.
    Admin to freely edit the content of the form fields and questions.

    form field types:
    drop-down where user is choosing one answer from the answers given by admin
    yes-no option as a form of answer
    user can answer with a custom text

    please share your thoughts.
