Custom Form for WordPress?

  I am developing my photography website using WordPress and I want to have a model application form with 3 image uploads. It would be nice to have it in the database so I can view online but from what I have read this isn’t possible at this time with WordPress.

  Is it possible to use custom code to create the form with the uploads and have the results mailed to my email? If not, will WordPress me to use a subdomain where I can house the form?

  The only form code on WordPress.COM blogs we can use is here > http://en.support.wordpress.com/contact-form/

  If that does not suit then you have to hire a web host http://wordpress.org/hosting/ and set up a WordPress.ORG install.
