customizing contact forms

Since you can’t upload files via ftp, does anyone know how to make a custom contact form. The plugin that wordpress has only allows the text fields to be entered directly below one another. Im trying to mimic something like this: http://uppermerionhomes(DOT)com/cma.htm

The good news is, the WordPress.com Business bundle you recently purchased comes with the Custom Design upgrade, and this might just be the answer you’re looking for. With the upgrade, you get pretty advanced tools to customize your blog’s appearance, including your contact forms.

Alternatvely, you may want to consider looking at Wufoo.com. It’s an online service that lets you create some really attractive contact forms that you can embed them into your WordPress.com blog. They allow you to create up to 3 forms for free.
