Database error

I recently created my website with 000webhosting and WordPress, now, when I log onto the control panel, it says :’ Welcome to the famous five-minute WordPress installation process! Just fill in the information below and you’ll be on your way to using the most extendable and powerful personal publishing platform in the world.’
Then it asks me to fill in information, when I do,:
