delete info from contact page

Somehow info got added into the name, email & website fields and stayed there! How can I delete it?
The blog I need help with is: (visible only to logged in users)

The visitor won’t see that on your site. If he/she is logged in, they’ll see their own information. If you’re logged in and visit a page with a contact form on a site other than your own, you’ll see your own information. But you’ll see it on a contact page on your own site as well. It’s not an issue. Best to just ignore it.
