Hi there,
You can achieve this by using a plugin, there multiple plugins available which might help & one of them is “form-forms”
Please click on this link to navigate to the plugin page: https://wordpress.com/plugins/form-forms/carlillyates.wordpress.com
Here is the link for live demo for this plugin form: http://rich-web.esy.es/wordpress-form-forms-job-application/
Please note you can only install plugins if you have a business plan activated on your account.
Please refer the below articles to know more about business plans:

Hi there, only people you add to your site in the Author role can upload their own documents to your site. Learn more about User Roles here: https://en.support.wordpress.com/user-roles/
There is no facility for visitors to upload documents. You might consider a shared folder on dropbox and suchlike instead.

@carlillyates – as @tarunvijwani mentioned, with the Business plan upgrade, you can install plugins on your WordPressdotcom site that can give you the ability to allow users to upload files. Here again is the link to the pricing page so you can compare our upgrade plans: http://wordpress.com/pricing
@tarunvijwani sorry, I neglected to refresh the page before submitting my reply. It’s all good. :)

Thank you both for taking the time and to advise.
Do you know if I can do this with the Premium package? I’m just starting and I am conscious of keeping costs down?
Thanks again -
Hi there,
You can only install plugins if you have a business plan activated on your account.
Please refer the below articles to know more about business plans:

https://en.support.wordpress.com/seo-tools/WordPress.com also allow you to cancel your plan, to get a refund, you need to cancel your upgrade plan within 30 days of purchase and domain registration with 48 hours of purchase.
You will be refunded to the same payment method you used for your purchase. After processing, refunds usually take about ten days to appear on your statement. For credit card refunds, it could take up to 1-2 billing cycles for the refund to show up. International refunds can take even longer to reach you. Your financial institution is going to be able to tell you when to expect the refund and how it will be listed on your statement.
Please refer this support article for more details for claims & refund:

Thanks!

Hi,
I really appreciate that information. Thank you very much for your help. I’m going to research and play with this.
I probably will have more questions in due course if that’s not too much trouble?

Feel free to ask away. :)
One thing you should determine is whether you want to add users to your site (as I mentioned in my earlier reply) or whether those uploading will be site visitors. That will better help you make a decision about plans.
Also, if you purchase any upgrade plan, you will have direct support by our Happiness Engineers by email or Live Chat.

Hi there,
You actually don’t need the Business Plan to do this. We support JotForm forms on WordPress.com, and those forms allow you to upload documents along with the form. The uploads won’t go to your WordPress.com site’s media library, but you’ll be able to access them via JotForm instead. You can use this on the free plan:
Please check if a particular feature is already available on WordPress.com or not before pushing users to plugins and the Business Plan/self-hosted WordPress. Telling someone they need a $299 a year upgrade for something they can do for free is not very good support.
Also, the plugin link you gave is to your own site’s dashboard and won’t be accessible to the OP. If you do need to link to a particular plugin, please use the WordPress.org/plugins URL for that plugin instead.

I only learned about it myself a few months ago, so happy to spread the knowledge around :)
