Does installing JetPack inform subscribers in Reader to follow you?

  I want to move my blog to a self-hosted site. I understand that a guided transfer will move the email subscribers but I wanted to know if the subscribers using Reader would be capable of still following me using JetPack.

  The blog I need help with is: (visible only to logged in users)

  Since that’s the case, does that mean that I have to send a notification (in whatever form that is convenient) to my followers of the new url?

  There is no “Reader” facility for blogs that are not hosted at WordPress.com, other than RSS readers like Google reader, etc.
