Does Premium Plan include domain mapping (not domain registration)?

I know that the Premium Plan says it includes ‘your custom domain’, but does that have to be one registered to wordpress.com? i.e. if we do the premium plan, do we have to pay extra if we want to map a domain registered somewhere else?
also, are any of the features listed in the plans limited to a certain time period, i.e. the plan only includes the domain cost for the first year and you have to pay for it separately after renewing?

You have to be logged in as Admin http://en.support.wordpress.com/user-roles/#administrator under the exact same username account that registered the blog to access the blog’s dashboard, cancel any upgrades, claim a refund for any qualified upgrades, and disable auto-renew.
