trying to find out if my domain was canceled when canceled my account yesterday

You must be logged in under the exact same username that registered the site to be able to access the site. The Admin login link to the dashboard for every blog hosted by wordpress.COM is:
http://NAME_OF_BLOG.wordpress.com/wp-admin/
Replace NAME_OF_BLOG in that URL with the actual name of your blog.
If you are not logged in under that account then clear your browser cache https://en.support.wordpress.com/browser-issues/#clearing-your-browser-cache and cookies https://en.support.wordpress.com/browser-issues/#clearing-wordpress-com-cookies and then login correctly under the relevant username account.

2. What is the underlying .wordpress.com URL?
3. Exactly which username account registered the underlying .wordpress.com blog and purchased the upgrade for it?
4. When was the upgrade purchased?
5. When did the refund request and upgrade cancellation take place? -
so, how can I tell if I still have the domain juliedossfitness.com? I want to use it at wordpress.org

When I am logged onto wordpress.com in my account, it lists juliedossfitness.wordpress.com under domains.
I had upgraded than requested a refund for juliedossfitness.com

Yes, the domain has been canceled. If you’re wanting to transfer your domain, you should never cancel it at your current host because once you decide to cancel your domain, there is no guarantee that you will be able to reacquire it in the future. Someone else could gain access to it. Also, it usually takes a few weeks for your domain registration to clear and become available once you cancel it. There isn’t an exact time, however, so you’ll be checking consistently to see when it becomes available.
If you want to keep using your domain but you don’t want to use WordPress.com anymore, you don’t have to cancel it. Instead, you can move the domain to another site
