Domain Mapping fee included in Premium fee?

  Hi
  I want to use domain mapping to bring another domain into WordPress. Is the $13 annual fee included in the $99 annual fee for the premium plan?

  Also, is there no discount if I have more than one premium plan.
  I assume it works on a per website domain basis, i.e. $99 per site?
