Domain Name

I upgraded to premium, but can’t tell if it went through. I’m still getting requests to upgrade.
I thought once you upgraded, the wordpress name automatically went off the domain name and it was just your page’s name. I’ve tried to change it automatically, but the page listing instructions of different types is so huge, it is all too confusing.
Have I truly been upgraded? If so, what can I do different on my page (print wise) besides get a different theme picture?

To check if you have truly been upgraded you can go to Dashboard -> Store -> My Upgrades. Upgrades
My Upgrades
You can manage all your upgrades across WordPress.com in one, convenient place on the My Upgrades page. You can access the My Upgrades page by going to Store → My Upgrades in your site’s dashboard. On this page you’ll find a list of the upgrades purchased for the site you are currently on, followed by lists of the upgrades you have on your other WordPress.com sites (if any).http://en.support.wordpress.com/my-upgrades/

1. But how do I change my domain name? I looked on a page packed full of lists, and can’t find how to change it. (Too much information?) Actually, I thought my domain name would change automatically and “wordpress” automatically dropped. If not, can you tell me how?
2. Also, what else is available to me for my webpage? I’m not yet into audio and video. Can I have a web page without a theme, and set up my own number of columns? And put pictures where I want them rather than across the top like the themes do?
