Domain Transfer Help Needed

  I recently decided to make the jump from wordpress.com to wordpress.org, my new blog is currently hosted at http://www.000webhost.com/

  I had registered a domain at wordpress.com a while back, my question is, how do I transfer the domain to my new blog? I added the name servers but it seems like their is more I need to do, I mean how would it recognize that is my domain just with the name servers?

  I found out a solution, I deleted my old account and made a new one, I wrote down my custom domain instead of using their free one and it looks like they linked, after changing the nameservers.
