E-mail form vs. Comment box

At the bottom of my page, there is an e-mail form. People who have tried to comment on my page end up e-mailing me instead. While that is nice, I would like to remove the confusion and have an e-mail icon rather than the form at the bottom that competes with the comment box.
Is this part of the theme I’ve chosen (Suits), or is it something I can alter?

In a related question, how do I include the little e-mail envelope icon in my sidebar to go to my e-mail address?

The blog I need help with is: (visible only to logged in users)

I just see a standard comment block at the bottom of your Posts. If you have comments set to “Moderate” you will receive and email for a comment that is waiting to be approved.

I don’t see any contact form or email you anything on your blog

    Thank you for responding!
On the main page, the only form at the bottom requires that you enter your name, email, website and “Comment” but that form just sends me an e-mail. You can only see comment boxes if you click on an individual post page – and then the comment box, which simply says “Leave a comment” is way at the bottom of the page, under the e-mail form I’ve described above. I think the confusion comes from the fact that the e-mail form says “comment”. I’m at a loss! Thanks again for your help!

    OK –

You have a contact form at the bottom of one of your Posts – http://thestorychanges.wordpress.com/2014/01/29/mother-teacher-writer/

There is also a standard leave a comment is also there. The contact form is at the bottom of your blog because the Post “Mother? Teacher? Writer?” is at the bottom of your front page

Edit that Post to remove the Contact form and put the contact form either in the sidebar or I put the contact form on a Page then I link to that Page when I want to have people contact me (the contact form is also in my top menu so anyone can find it easily)

Thank you for the link to “Contact Form” support – that did the trick for removing that form!
