editor icons missing in HTML editor

On one of the site I manage, the editor icons have suddenly stopped appearing at the top of the editor field in the HTML editor. They do show up in the visual editor, but I prefer not to use that editor because of how it can rewrite code.

I’ve re-installed WordPress 4.1 but it didn’t fix the problem. I’ve turned off all the plugins and it didn’t solve the problem. I also tried checking if it was browser-specific but it happens on all browsers on that site only. Any ideas what’s causing this or how I might resolve it?

Though you failed to post the URL of the site in question it’s clear you are not referring to a WordPress.COM hosted blog. That site is not hosted by WordPress.COM. Our support docs do not apply to any sites that are not hosted by WordPress.COM and we do not provide support for them. You are posting to the wrong support forum. We cannot help you here.

WordPress.COM and WordPress.ORG are completely separate and have different logins, features, run different versions of some themes with the same names, and have separate support documentation and separate support forums. Read the differences here http://en.support.wordpress.com/com-vs-org/

If you don’t have a username account at WordPress.ORG click http://wordpress.org/support/ and register one on the top right hand corner of the page that opens, so you can post to the support forums there.
