There is no FTP access to wordpress.com blogs and we cannot upload forms from elsewhere into our wordpress.com blogs.

http://en.support.wordpress.com/code/It’s important to know the differences between wordpress.COM free hosted blogs and wordpress.ORG installs as they run on different software.
http://support.wordpress.com/com-vs-org/We do have a contact form that we can use on our blogs regardless of the theme we use and the link to instructions is this one > http://en.support.wordpress.com/contact-form/

HI, thank you for the clarifications. I have tried to insert the contact form as described above, but when I publish it, the fields are filled in with my personal wordpress account information (name, email, and website). I can’t seem to get it to publish with blank fields! What could I be doing wrong? The code is exactly what is given in the example – I have not altered anything.

@art925, that is because wordpress is seeing the cookie it has placed on your computer, so it knows who you are and will fill that information in for you. Create the contact form and publish the page. Log out of wordpress, clear your browser cache and cookies, restart your browser and then, without logging into wordpress, visit your site and you will see they are blank. Anyone else visiting your site will either see their information (if they have commented on a wordpress.com blog and have a similar cookie on their system, or they will see blanks if they have not.

What you see is your own information because you are logged into wordpress.com. No one else can see it. Log out and you will see what I mean. WordPress.com uses cookies to keep track of commenters and those filling out contact forms so if a wordpress.com member is logged in and submits comment or fills in a contact form on your blog, the next time they come to your blog, unless they have cleared their cookies, WordPress.com will recognize them and complete the details for them. No one else will be able to view those details. The next person who comments will see blank fields. Commenters will not see each other’s details.
