Email notifications from the custom form on my blog are not being delivered?

  Why don’t my email notifications work? I set up a custom form on one of the pages and I would like to be notified by email whenever anyone fills out the form. But I tested the form myself, after signing out of my WordPress account, and submitted the form several times, but I don’t get notified.

  The blog I need help with is: (visible only to logged in users)
