Email Subscription Form 'Fly-in"

  Hi,

  I would like to know if there’s a plugin for an email subscription which allows a fly-in effect. Something like at http://www.themillionairefastlane.com at the beginning.

  It’s something like a form at the beginning where I can provide a contact form to give away a free ebook.
