Facebook Pixel Not Firing

  Hi guys! I installed a facebook pixel to my site & from the looks of the Facebook Pixel Helper its there but not working. I inserted the code in the php file of the header. I also tried the ‘Insert Header & Footer’ plugin but that didn’t work. I also tried the Facebook Pixel WordPress integration app. None worked what should I do?
