feedback form?

  trying add the feedback form to my blog but everytime when i try to add and view after adding as new is there something wrong i am dooing cant seem to make feedback form. it post the html in then i press publish but it not adding visually what happening?

  If you are using the contact form here, try switching to the HTML tab in the editor before adding the form. It shouldn’t matter, but sometimes it does.

  ok that problem sorted but now getting my info asppearing in form how do make it blank still with subject matter to my email ? this wierd still new to this

  The info appears because you are logged in – nobody else can see it – If I visit your site while logged in I will see my own info in the form – it’s a “feature” that freaks people out on a regular basis – you only see your own info –
