Feedback form doesn't show

The feedback form I put in a post only shows on my blog url, not on the individual post url. How can I make sure it shows in an individual post?

All WordPress.com blog have comments open on all pages and posts unless we choose to disable them. This page > Settings > Discussion settings is where you set up defaults for comments. http://en.support.wordpress.com/settings/discussion-settings/ You can also override the default settings and enable or disable future comments on any post or page. http://en.support.wordpress.com/enable-disable-comments/
Most bloggers place a single contact form on a static page rather than on each post as there are already comment boxes. However, if you wish you can use the same guide to embed a contact form into each of your posts. http://en.support.wordpress.com/contact-form/
