Feedback form/user-submitted posts

We would like to include a way for users to fill out a form or survey and have that information appear automatically on a certain webpage (pending approval). We like the User Submitted Posts plugin, but apparently can’t use it on a WordPress.com-hosted site. http://wordpress.org/plugins/user-submitted-posts/

There is no FTP access to free hosted WordPress.com blogs and we bloggers cannot install any plugins.

FTP access, plugins and third party themes are available only for VIP bloggers http://vip.wordpress.com/our-services/ or for those with WordPress.ORG installs.
WordPress.com and WordPress.org are completely separate. if you require plugins then you must hire a web host http://wordpress.org/hosting/ and set up your own WordPress.org install. http://wordpress.org/
