Feedback or suggestion box for my blog

  Hi! I want to put a feedback or a suggestion box on the side bar of my blog. It could be anonymous if the reader didn’t want a personal response back. I wouldn’t want it to show up on my blog. These would just be for me to help me generate new blog ideas and topics.
