  I want to create a feedback page on a free hosted WordPress.com blog.

  I want the page to work something like how the comments work but perhaps allowing a couple of fields to be on the page eg asking for someone’s mail address or their request to us or their phone number…that kind of thing.

  I appreciate it probably CAN’T be done…but just asking on the offchance that someone has thought of a clever workaround for this – which many small businesses and some bloggers would need/like.

  If it can’t be done in a page is there some way of doing it with a text widget ?
