FORGOT E-MAIL, USERNAME AND PASSWORD

I haven’t used my word press site in years, nor have I logged in to my website. This has become increasingly irritating. I simply want to shut down my website, because it has my name in the header.
If anyone can help please e-mail me @ (email visible only to moderators and staff)
I dont want to put the name of the website on the wordpress forums, as it will just add another search result to google.FORGOT E-MAIL, USERNAME AND PASSWORD
