Form and database plugin

Hello
I have a database with 500+ members containing Name, Adress, member ID ect.
I need a plugin where i can write or select a member via a dropdown meny using the member ID and/or name and when that’s done their additional information will appear.
Below that info i need a coupe of imput fields where i can write some info and when i press a button that info will be inserted into another table where the member ID will be the key.I have been looking at Contactform 7 and their condentional fields, but it seems that it’s can’t pull the info from a database.
Any pointers?

Do you want to use a membership plugin on r66dk.wordpress.com? If so, you can search for available plugins here:

It’s worth noting custom plugins can only be installed on WordPress.com sites using the Business plan. I don’t have a specific recommendation but if you want to charge for memberships, WooCommerce Memberships may be suitable:

Thanks for the reply.
I guess I havn’t been too good explaining what it is i need (and where)
1. I have my own domain where my website is located, and that supports wordpress
2. On that domain i have (or will build a database depending on how i fix the next problem) with all my members name, adress, phone number, e-mail, and member ID
3.1 I need a form (or something) where i can punch in the members ID number and it autofills the name, adress and phonenumber into the rest of the form based on the info in the database.
(much like Googles adress lookup)3.11 Best case i can punch into any field (name, adress) and it will autofill the rest of the form according to the database
4. I have a blank field on the form for messages.
5. When I push send, it sends the entire form to the e-mail and/or saves the message into another database so i can look back and see who got what messages and when

It sounds like you have a site using the open source version of WordPress. We don’t provide support for that version of WordPress here in the WordPress.com forums, and we also cannot recommend specific plugins, simply because neither the staff nor the volunteers in this forum have sufficient experience with the plugins available for the self-hosted version of WordPress to make recommendations like that.
Support for the open source version of WordPress is provided by the community over at https://wordpress.org/support/, and that’s the best place to ask for help with something like this.
