Form before viewing website.

Which plugin could I use to enable a pop-up or page to appear before viewing the website. Without that info filled the user cannot view the site.
The blog I need help with is: (visible only to logged in users)

There is no FTP access and no blogger installed plugin capability so that is not possible on any wordpress.com hosted blog. The only way to achieve it is to make the blog private. To change blog visibility to Private go to > Settings > Reading scroll to Blog Visibility and choose option 3. See the guide here http://en.support.wordpress.com/settings/privacy-settings

WordPress.COM and WordPress.ORG are completely separate and have different username accounts, logins, features, run different versions of some themes with the same names, and have separate support documentation and separate support forums.
