Form Block option and feedback category in dashboard area are both missing

  I am trying to add a simple contact form to my website, I have no option for the form block. How can I get the WordPress form block option to appear?

  Hi there, both the form block and feedback tab are part of Jetpack which is in use here on WPcom.

  If you have your own install of the WP open source software with another hosting service, you would need to install the jetpack plugin https://jetpack.com/
