  I tried to insert a contact form on my blog using form builder but every time I insert it my personal contact info shows up in the “required fields”. How do I prevent this?

  You don’t.

  You filled in that information, and the computer is remembering it for you and pre-filling in those forms. That information’s not visible to anyone else, don’t worry.
