Form builder: get the current post attributes

Hello all,
I am a newbie to wordpress. I want to integrate forms in my posts with a special requirement: at the form submission, I would like to know from which post the form was submitted.
I have tried one form builder plugin called “Form Maker”, but didn’t find an obvious way to get this data. I have read a bit about other form plugins, but this feature is never indicated.
Is there any form plugin that satisfy easily my requirement? Otherwise who can I twist it to be access the data about the current page from which the form is submitted?
Any hints are welcome.

There is no FTP access to WordPress.COM blogs like and we bloggers cannot install any plugins.

A self-hosted WordPress.ORG site might be right for you. WordPress.org offers free software that you can install on a web server. actually I am already using wordpress.org.
We don’t provide support for any blogs other than those hosted by WordPress.COM and that site is not one of them.
WordPress.com and WordPress.org are completely separate and have different logins, features, run different versions of some themes with the same names, and have separate support forums. http://support.wordpress.com/com-vs-org/
If you don’t have a username account at WordPress.ORG click http://wordpress.org/support/ and register one on the top right hand corner of the page that opens, so you can post to the support forums there and receive advice from WordPress.ORG bloggers.
