  After downloading 23 (!) form creators from WordPress and not having success with any of them, I went outside of WordPress to find one. Wufoo did 100% of what I wanted to do, easily and free. Why can’t the WordPress form creator plugins be this simple? You can find Wufoo at http://www.wufoo.com/
