In case you weren’t aware of it, we can currently use Wufoo and Jotforms.

Jotform needs to be embedded with an HTTPS link in order to show up in WordPress.com. If you don’t use HTTPS, it just appears as a hyperlink. Jotforms even allows you to link to a Paypal account and include payment options within the form, which is very cool.
Here are more details from the Jotforms website:

Thank YOU, @justjennifer, for putting us on to Jotforms in the first place. I wouldn’t have known about them if I’d just searched Support for forms!

I would like to have a plugin for quizs on my blog.
Like this for an example:
> easy Quiz:
Install the plugin on your WordPress page. Or upload it to your FTP in the /wp-content/plugins/ directory
Activate the plugin through the ‘Plugins’ menu in WordPress
Now you should have a new menu in the admin called ‘Easy Quiz Player’. Go there.
Create your free account and start creating your first quiz. Enjoy! -
To quote @jeremeylduvall,

PollDaddy is built into WordPress.com. Perhaps it would be useful for your quizzes. Here’s some more information about using polls on WordPress.com
