Form responses not being sent with notification emails

Hi there, I have put together a form for our members to make certain requests. These requests are set up to be sent to two people in our management. They are getting an emails when someone submits their form responses, but the emails are blank. How can I make the responses come through in those emails? Thanks.

Hi, nope it’s not that website and not a contact form. This is for http://www.sfgrotto.org, and it’s an internal form not visible to the public. I created it by going to the “forms” section in the left nav bar. I can send a screenshot of what I’m talking about but I don’t see a way to include an image in this reply.

It seems you’re in the wrong forum.
I checked and found that your website is using name servers that aren’t associated with WordPress.com.Name servers:
NS1.WEBSITEHOSTSERVER.NET
