Form submission

    Any advice would be appreciated.
    I’m trying to create a form (maybe I don’t need a form?) – what I’m after is this.
    I want the user to select 1 of 3 options, based on their selection, another list of options appears, and based on their selection there, a 3rd set of options appears.
    Then I want to grab their 3 selections, along with their contact info and submit that in an email.
    How can this be accomplished?
    Example:
    Pick a Color (Red, Blue, White)
    If Red, have them pick a #: 1, 3, 5, 7, 9
    If Blue, have them pick a #: 2, 4, 6, 8
    If White, have them pick a #: 0, 10.
    If 1,3,5,7,9 – have them pick a day: Sun, Mon, Tue
    If 2,4,6,8 – have them pick a day: Wed, Thu, Fri
    If 0,10 – have them pick a day: Fri, Sat.

    Then I want to submit “Red, 5, Mon” along with their contact info.

    Hope that makes sense.

    This type of form is not possible on WordPress.com. What you’re describing is a survey/polling option rather than just a form.

    You can set up a survey like that using Crowdsignal.com (you can sign in there using your existing WordPress.com account), and then you can link to it from a post on your site. It can’t be embedded directly on your site, though.
