Form Submission being blocked by firewall

  HELLO!

  1. We embedded a formidable form on our WordPress site and published it: https://www.uscold.com/carriers/
  2. When trying to access the page, we are getting an error message:
  The transfer has triggered a Web Application Firewall.
  This transfer is blocked. URL: https://www.uscold.com/carriers/
  Event ID :
  Event Type: method restriction
  3. Our web hosting provider is asking for the “embedded URL for where the form is being submitted” so they can whitelist it.

  Where do I find the URL?
