  Hi, I already included the form in my page, but when I preview it, the form was prefilled with my details? How to remove my details?

  The blog I need help with is: (visible only to logged in users)

  Hey lovelynmontepio,

  Not to worry! The form pre-fills in your details because you are the one looking at it. If another visitor if viewing your page, it will pre-fill with their details.
