Formatting toolbar disappears

I am trying to add a new post but when I click on the visual tab there is no formatting toolbar. The HTML tab toolbar does appear for a split second but then disappears to leave only the media upload icons. Also any text I type in the visual tab appears as html code. Really not sure what to do and would appreciate any help!
