Forms for Non-Premiere Themes

  Are there any themes that are not Premiere that provide a contact form with real input fields and a SUBMIT button?

  If so, please direct me to one.

  You can use a contact form on any theme, in a post, on a page, or in a widget. You can also decide what appears on the form’s fields. See here:
