Forms – how to stop grouping and pre-filling of fields

I’m new to WordPress and just getting started with my site. I’m creating a registration form for people who will sign up for the classes I’ll be offering. I started with the contact form and made edits to it to create the reg form. I’ve experienced a couple problems:
    1. The fields sometimes group themselves together, so if I try to change the name of one, it also changes name of the one below or above it and if I try to move one, it moves two or three of them at a time and I can’t move them separately.
    2. When I go view the form on my site, some fields are prefilled with my own info, like the email field has my email in it (I want my customer’s email there) and the first field, which is for name of the class, is prefilled with my WordPress user name. How can I get rid of these?

The blog I need help with is: (visible only to logged in users)

  • Regarding point 1: if you have trouble editing a contact form, try clearing your browser cache and cookies: http://en.support.wordpress.com/browser-issues/ If that doesn’t work, I’d recommend deleting the form completely and inserting a new one from scratch – it could be that when you tried to edit the form a bit of code got left behind. Creating a new form would resolve that.

Regarding point 2: that auto-fill info is stored and completed by your browser. If you look at a contact form on any blog you’ll see your own info there. When other people view your form they’ll see their own information, not yours.
