Forms not appearing on page

  Hi

  I am using the Great theme and I noticed it is not displaying any of my ninja forms. All I see on the page(http://www.baby-daycare.co.za/contact-us/) is

  /*
  Contact Me
  /*]]>*/

  If I change to another theme the form appears.

  The blog I need help with is: (visible only to logged in users)

  These forums are for sites hosted on WordPress.com, but you’re running the WordPress software on your own hosting. If you need further help with the self-hosted version of your theme, you can try getting help at WordPress.org:
