  Hello, I am looking for a free custom plugin that will allow me to add more than 5 fields without paying for an upgrade. I’m looking for a plug in that will allow me to create forms that are not contact forms, but of many other customized forms.

  The blog I need help with is: (visible only to logged in users)

  We cannot use plugins on WordPress.com.
  The only contact form is the shortcode as described her:
  http://en.support.wordpress.com/contact-form/
  It can be customized with more than 5 fields.
