Generate report on the basis of form data

  Hi

  So I’m a psychologist and I’m trying to make a form on my wordpress site (Self hosted) in which the user will fill the answers and each response will be calculated based on the set parameters (Like True = 1 point, False = -1 point; or Option 1 = 1, Option 2 = 2, Option 3 = 3, and so on)

  At the end of the test, there will be a buy button so that the user can buy the test results (as a PDF). Maybe the buy button is linked to woocommerce.

  Is there any plugin available to do that? or something similar that can be customized to do the needed?

  Any lead would be appreciated.

  Looks like you’re in the wrong support forum. You use the self-hosted version of WordPress rather than WordPress.com.
