Have you allowed a domain mapping upgrade to expire? Some time ago now (months ago) individual domain upgrades were discontinued. Only those who had domain mapping upgrades that have not expired are allowed to renew at the same rate. All others have to add a plan http://store.wordpress.com/plans and purchase an upgrade bundle. https://wordpress.com/pricing/
You have to be logged in as Admin http://en.support.wordpress.com/user-roles/#administrator under the exact same username account that registered the blog to access the blog’s dashboard, renew or cancel any upgrades, claim a refund for any qualified upgrades, and disable auto-renew. Dashboard > Store > My Upgrades.
If you are not logged in under the correct username account clear your browser cache https://en.support.wordpress.com/browser-issues/#clearing-your-browser-cache and cookies https://en.support.wordpress.com/browser-issues/#clearing-wordpress-com-cookies and then login under the correct username account.

3. Exactly which username account registered the underlying .wordpress.com blog and purchased the upgrade for it?

Hi @samsunggearvrreview, you wouldn’t have had a $13 option. But you can go ahead and get the personal plan here:
https://wordpress.com/plansIt comes with a domain credit, direct support, and it removes ads from your site. Once you’ve bought the plan, just click on your “Claim Domain” message, or head over to https://wordpress.com/domains to set up the mapping as you’d like it.
