Getting rid of an account/site

  Hi,
  I am trying to delete an account and the WordPress website I created with that account (if it helps, I also did domain mapping etc with WordPress on this site). In my Account Settings, under the Primary Site drop-down menu, the same site is listed there twice with a lock icon next to it. I want to get rid of this site (both of them – not sure why there are two of the same) and then this account as I no longer use it and want to use this email address for a different, future account.
  How do I remove it all?
