Google Form embedded on Mac

I have implemented a google form on my ‘rentals’ page of the website, and it shows up on every kind of computer except for Mac’s. We are using google chrome, yet I’ve used google chrome on other devices and it works there, so I am not sure what is happening. Here is the link https://valpoupc.wordpress.com/rentals/ .

I am currently on a mac. The message I get when displaying the form is as follows:
You need permission
This form can only be viewed by users in the owner’s organization.Try contacting the owner of the form if you think this is a mistake. Learn More.
It appears that the permissions for the form are not set to public.

You’re getting this notice because of the settings you have on your form. At the top of the form is “require sign-in” for the form checked? If so, uncheck it. Also, make sure you don’t collect log-in info, as that will automatically restrict outside of your domain.
