Google Form not working

  I embedded the google form iframe code. It worked.

  Then after a few times of saving, it stopped working. (I did not go back and forth between Visual & Text. I stayed on Text.) After playing around trying to fix it, I deleted the page and started again. It never worked after that.

  I assume you can see what the code was changed to after I saved?
