A wordpress.com account and a gravatar account are one in the same thing.

Gravatar/blavatar (favicon) images do not show up instantly after uploading them. Gravatar images are cached and frequently take several days to display after a change.

Dashboard → Settings → General
* Replace “http://INSERT_NAME_OF_BLOG” below with the actual name of your blog in the URL

Do not keep uploading!
Do not forget to select an image rating.

WordPress.com and WordPress.org are different but you can use Gravatar for you self-hosted site as well:

You can follow these steps to use your Gravatar on your self hosted site:
1.) Login to your site’s Dashboard, and navigate to Settings > Discussion.
2.) Scroll down to the section titled Avatars, and make sure “Show Avatars” is checked.
3.) Save your changes.
4.) Now, navigate to Users > Your Profile.
5.) Scroll down to Contact Info, and make note of your email address.
6.) Confirm that you have a Gravatar.com account associated with that email address. You can use this tool to check:
http://en.gravatar.com/site/check/7.) If you don’t have an account associated with that email address, head over to http://gravatar.com and create one.
8.) Now your Gravatar will appear with your author account!
