Here are some brief answers to your questions:
1. At least a Premium plan, ideally a Business plan when getting big. A Personal plan would be the bare minimum which will offer you the option to have a custom domain.
If you want to run Google AdSense ads and/or other ad networks like BuySellAds, OpenX etc., you will need to be on at least the Business plan.
The Premium plan supports WordAds – which is an ad system managed by WordPress.com that will serve ads from networks selected from WordPress.
Premium content / subscriber-only content is available even on the Personal plan. (But you can’t run ad networks on the Personal plan).
Also, Premium and above plans allows more control over themes and styles, which might be handy/necessary when approaching blogging as a business.

2. Yes, unless WordPress changes their pricing.
3. I’m guessing that you don’t want a setup with a static homepage and additional pages and instead you want to display your blog posts.
For this, go to the “Customize” page of your website/blog (https://wordpress.com/customize), select the “Homepage Settings” option and under the heading “Your homepage displays” select “Your latest posts”.Do these answer your queries?
