Does purchase of the guided transfer include transfer to a self-hosted site at Hostmonster or only to the hosts you have listed as recommended?

I checked here http://en.support.wordpress.com/guided-transfer/ and it says has a link to these recommended hosts but the host you name is not among theme. -
Yes, I saw it was Hostmonster was not on the list but I wondered if they would provide help with hosts other than those recommended. I guess not. (I previously paid for Hostmonster).

Thank you timethief. I had another question after reading that post and the recommended links she included: one of the posts says all content and comments, as well as the email notification feature can be transferred, but that once you move to wordpress.org, you are no longer part of the wordpress.com community and you lose the “like” function as well as being included in the reader feed. Is this true and how might that affect my existing readership?

It is partly true. I believe you lose the Like function, but you still appear in the Reader IF you install Jetpack and have staff here port your followers over.
You will, by moving to a smaller host, lose SEO and your hits will go down. This is inevitable in moving from a huge to a smaller host of any sort and there’s nothing you can do about it except post stuff people really want to link to in the new site.
